A nine years old girl fell 18 m (app. 50 feet) to her death. She was caught by the belay rope after 2 meters drop, and then suddenly continued falling to the ground. Reasons unknown.

Given the mechanical translation on Google maybe this will be useful

The key paragraph in this report is

There are two possible scenarios. First, the father lost control of the rope. Second, The knot, which attached the (safety)rope to Sandra’s harness failed (became undone). Andreas Feile said, “As she lay on the floor, the rope was not attached to her harness. He was one of the first in the gym to reach Andrea. He declines to say more. The gym was closed on Wednesday.

Also, of significance.

“ Niemand weiß, ob er Klettererfahrung hat.“

No one knows whether he (the father of one of the girls who had taken them to the gym) had climbing experience.

damn, thats sad.

imho the most likely scenario is that the knot came undone. im not sure, but the comment about the rope not being attached to the harness suggests this possibility rather strongly.

also they have checked all the gear and it was 'safe'.

Most probably she was not tied in properly.
Either the knot was not right or not finished.
