What happened to the "upload" attachment button?

When replying to or creating a post, there used to be an "upload attachment" and "browse" button, but they are now missing.

What happened to them?

OK... everything is back to normal!

FWIW, the issue was that we've reached our global maximum for total number of attachments in the database. We've just increased the cap for now.
