Looks fake. This guy waited until he was indoors and had taken a picture before taking this out? Not likely.

Don't know if it's fake or not, although the idea of impaling oneself in the upper arm like that seems unlikely. OTOH, my partner and I once rescued a beginning leader on Grease Gun Groove in the Gunks who had fallen and grabbed the quickdraw, causing the biner to impale his hand in the web between the thumb and index finger (Ouch!). Lucky for him, he ended up hitting (without additional injury) a ledge at about the same time, so he didn't rip it entirely.
Ironically, I made my first trad lead on GGG (in retrospect, a really stupid choice: too many ledges to hit if you fall, which I, as a novice, thought would be good).

The MP thread says that he couldn't take the biner out because of the hook on the nose of the biner for the gate. Makes sense to me. He also said that he taped the biner open after the fact to make it less uncomfortable.
I don't know if it is fake or not, but it certainly looks painful if it is real.

Two related incidents I know of to be true:
* outdoor sport climbing (I personally know the victim): leader fell before second bolt, probably grabbed at draw on way down (ground was close), and impaled wrist on draw biner - 9 stitches? At least part of weight was supported by draw.
* indoor sport climbing (my son worked at the establishment): leader fell and hooked achilles tendon on draw biner; left hanging upside down by it and had to be lifted off.
I'd guess these kind of incidents occur about as often as a trad leader falling and getting hung up by their shoulder-slung rack.
