I rack biner gate facing out. That means that the cams get racked however they happen to be sitting on the biner at that moment. I'd be willing to bet that around 99.99999% of climbers don't care in the slightest what way their cams are racked (in the context of your question), and I don't know about your cams, but mine certainly rotate and move around as I'm climbing.

The obviously correct answer is to alternate. That way they rack closer together and thus take up less space on your harness.
