From the accident report:

It sounds like it - yet again - was an accident where the climber expected to be lowered, but actually was off belay.

I disagree. The reporting you quoted is NOT very technical. In context it seems the reporter was simply saying they either "fell" or otherwise weighted the top-rope for a rest.
Though I could have missed something.
