Unfortunately the article doesn't give any indication whatsoever if it was equipment failure, incorrect set up (ill-fitting harness, incorrectly tied knot, etc.) or just plain ol' belayer error.

The odds are almost entirely in favor of belayer error, as are most gym-related injuries, but again, the article doesn't give any clues to the real cause.

I hope for a speedy recovery for both the injured folks in the article.

I hope he recovers from his injuries without long term problems. From the story it sounds like he was a beginner who was belayed by an instructor. Details in the story suggest the instructor tied the knot, and then encouraged his student to sit back in his harness to be lowered off when he became tired.

Very sad.
