Top-out, no question. Even if all the top-outs are easy, they still help get the top-out moves wired. I also feel that even easy top-outs help a ton with flexibility. If people don't want to top-out, they can drop off after matching the last hold, or whatever.
I have yet to see a gym that requires topping out straight from an overhang (heel-hook -> rockover type moves.) That would be immensely helpful. Most gyms put a 1-3 foot slab to top out on, which is still good, but there are a lot of tricky top-outs outdoors which that doesn't prepare you for.

the gym in charlotte nc has a steep section straight to top-out. It really does help with real topouts.
