Onsight ascent by:

A hold broke off as I was mantling onto the ledge with the chains. Belayer wasn't looking, and I took a 20 footer. As a bonus, rope became severely jammed, and I had to unclip pieces and downclimb to rescue the rope. Heck of a time.
