We had an epic on this one: my lead partner fell when finishing the pitch (the screw held fine), climbed back up to the screw to find that ice around it cracked rather badly. He then tried to place another screw (his last one) and make a belay station. Well, he dropped thay screw. He then found himself a good stance a decided to find a new placement for the screw that held his fall. Needless to say, he dropped that one too. He then decided that he really needed something in his backpack. Do I have to say that he dropped that backpack as well? :o) His next screw was now smth like 12 feet below him and there was a real nice ledge some 5-10 feet below that screw... Well, it's only a third of the story but the rest would take too much effort to type:) I'll just add that we did finish the technical part whole and alive but still had to dig an ice cave next to the summit to spend a rather cold night...
