It's a typical news report of a climbing accident - scant on actual info and probable incorrect use of climbing terms; eg: it's probably not an anchor that blew and he was likely soloing since there's mention of friends watching from below but no mention of a partner. Regarding the other two questions, we'll probably never know those answers unless the injured climber comes forth with his story.
